The test does run 2 processes. I tried this:

$ cat test.c
#include <mpi.h>
#include <stdio.h>
int main(int argc, char *argv[]) {
	int rank, size;
	MPI_Init(&argc, &argv);
	MPI_Comm_rank(MPI_COMM_WORLD, &rank);
	MPI_Comm_size(MPI_COMM_WORLD, &size);
	printf("I'm %d/%d\n", rank, size);
	return 0;
}

And it reports:

$ mpirun -np 2 ./test

and the same with a hosts file containing localhost twice.
