The upgrading stylesheet omits titles in qandaset, because there is no
special-case handling for qandaset and therefore the default behavior
for title is to ignore it.  The stylesheet should preserve these.

This is a problem for me since I am updating some stylesheets that work
with DocBook 4 to DocBook 5.  Part of the automated testing I do
involves upgrading the DocBook 4 testcases (using db4-upgrade.xsl) and
testing that the output of my stylesheets is the same.
