Yes.

> 2. what does /proc/mounts say when it's failing to umount?

/dev/mapper/cr_hda7 /var/cache/pbuilder/build/5906/tmp/pbuilder-shared ext3 rw,nosuid,nodev,data=ordered 0 0
/dev/mapper/cr_hda7 /var/cache/pbuilder/build/5906/tmp/.X11-unix ext3 rw,nosuid,nodev,data=ordered 0 0
/dev/mapper/cr_hda7 /var/cache/pbuilder/build/5906/tmp ext3 rw,nosuid,nodev,data=ordered 0 0

... which is weird. /dev/mapper/cr_hda7 *is* my /tmp, but these
don't really resemble bind mounts really. the /bin/mount output is
right though.

> 3. is it reproducible all the time?

Yes.
