As discussed with Daniel T. Chen (crimsun) on #pulseaudio at freenode,
there seems to be an underlying bug on alsa that makes pulseaudio
volume control introduce various glitches and noise to whatever's
being played.  Passing 'position_fix=1' to the module as suggested
works around the problem.
