/etc/default/clamav-milter is not shipped in the new package anymore, none of
the above options are available as command line parameters anymore. Instead,
check /etc/clamav/clamav-milter.conf.

Hmm, yes, known bug. Sorry.

You might want to edit /etc/clamav/clamav-milter.conf instead.

I think the AddHeader option of /etc/clamav/clamav-milter.conf should do the
trick. It's not as verbose as the old one, but you'll get a header at least.
