Sure. I don't use the daemon mode, just the command line agent. Here is an
excerpt from my exim configuration:

begin routers
# ...
spamscan:
  no_verify
  condition = "${if and {{!eq {$received_protocol}{spam-scanned}} {!eq {$received_protocol}{local}} } {1}{0}}"
  driver = accept
  transport = spamcheck
  require_files = /var/lib/dspam:+/usr/bin

  user = dspam
  group = dspam
  return_path_add = false

No, it is not enabled. I guess the most important bits of the config (though
you're welcome to the entire thing if you want) are:

Preference "spamAction=quarantine"
Preference "signatureLocation=headers"  # 'message' or 'headers'
Preference "showFactors=off"
