Using the same evolution-exchange version on unstable, on that screen, I have
two fields:

Username:
OWA URL:

Whatever I type in the Username: field, it's not enough for the plugin to be
configured. I have to type something in the OWA URL, and I guess I have to
check with "Authenticate" before continuing.

I don't have an exchange server available here so I can't help you more, but I
guess you should try to enter that url.
