I try zo run rcim4 in a configuration, where local delivery is always
on, but smarthost delivery is only when ppp comes up.

I think this should be the queuerunnermode ppp.
But it does not start the local delivery in /etc/init.d/exim4

Exim doesn't distinguish between local and remote deliveries. It
automatically tries to deliver a message immediately if local load
control restrictions and configuration allows (we use all default
here). If that delivery fails, the messages are queued, regardless of
local or non-local.

queuerunnermode ppp starts a queue run when a new PPP connection is
built.

To me, it looks like exim does perform exactly as intended. Closing
the bug. You're free to  re-open and to substantiate your
argumentation that the behavior is indeed a bug.
