This bug report has evolved into an unmaintainable horrible mess. A
lot of the issues mentioned herein may be unrelated to each other.

The different issues listed in this bug have been moved out to
dedicated bug reports (#459323, #467136, #467137, #467138, #467151,
#467152, #467158).

Please look at these bugs only and only add information if you are
sure that you are experiencing the same issue that is mentioned in the
respective bug report. If in doubt, please file a new, unrelated
report.

Messages added to #348046 are likely to be ignored as I do not intend
to look at this horrible mess again before all blocking bugs were
resolved.
