Well - MailScanner offers IMO most flexibility concerning what to do
when the AV scanner finds malware etc.
At our department, we don't want to miss it.
Maybe that other wrappers (e.g. amavis) offer as much flexibility, but
MailScanner satisfies our needs.

MailScanner doesn't break anything. But IMO there is some more work
necessary to get it running on Debian.
You need two instances of exim which AFAIK makes it necessary to
change init scripts, pid handling, create a second spool directory and
so on.
As this fairly differs from the "stock Debian configuration" of the
exim4 package, I recommend creating your own configuration files.

The MailScanner docs drop some words on how to configure the two exim
instances...
