This could be anything. Without access to your server no one would be 
able to give you a definite idea of what is going on. But I'd suggest to 
(temporarily) simplify your setup so you can eliminate that part as a 
point of failure.

Make a backup of your current configuration and remove the spamfiltering 
and other tweaks and just have a vanilla exim configuration using 
dpkg-reconfigure exim4-config. I'd also suggest to actually put the 
spamfiltering on a separate server.

Using lvm is a bad idea if you need redundancy. For example using 2 
disks to simulate 1 disk doubles the chance of failure causing all data 
to be lost.
