As soon as the => line appears, exim has successfully delivered the
message to the host mentioned there, here "smtp.west.cox.net", and
received the remote host's confirmation that it has taken
responsibility for the message.

I cannot conclude what happened to your message, but it didn't happen
on your box. Your exim did its job just fine.

I can only guess that cox.net (or some system further downstream)
doesn't like your envelope sender holtzm at localhost and silently drops
the message.

You may want to install the swaks package and test which messages are
actually delivered and which aren't.
