Errrr... I could be wrong here, but doesn't Debian Policy 3.8.0
require a README.source file when any patches are applied during
the build procedure, as with dpatch, quilt et al?  At least, that's
the way I understand Policy 4.14; of course, dpkg-buildpackage will
invoke "./debian/rules build", which ought to depend on "patch" in
some way, but still, that's the way I've seen it done in lots of
packages over the past several months since Policy version 3.8.0
came out...
