I've started looking closer at NetworkManager and what I read seemed promising.  So, I
pulled it in to try.  Unfortunately, madwifi absolutely will not get a dhcp address
when network-manager is driving it.  I can see dhcp discover messages in the log
as I expect, but no lease.  If I disable network-manager, I can ifup ath0 and it
works correctly.

This is an open unrestricted network connection, so no WEP or WPA surprises should be
involved.
