Tried it again, this time with a fresh profile. No difference, the login still fails. Ctrl+Shift+A shows no extensions and/or plugins.

BTW, my problem only appears at a particular bank- I browsed through underlying html but didn't see anything obviously wrong with the login form there. Haven't observed any problems with other secure sites.
