The name in the changelog should match what is in debian/control
Maintainer of Uploaders field.

Usual practice is to upload -1 revisions as first revision instead of -0.

Your changelog has not been updated and your declared Standard-Version
is newer than the date in the changelog. Update the changelg with dch.

git-dch is only useful when you are using git (it generates changelog
entries using the git commit messages). Also, first uploads rarely
have to include an extensive changelog, since "Initial release"
usually covers all necessary information.
