[php-maint] Bug#392249: php5-common: at least users should be cautioned in README.Debian

Followup-For: Bug #392249

  When add/remove php5 extensions(not only php5-mysql), apache should be 
restart to apply changes, so at least users should be cautioned this in 
README.Debian.
