According to pulseaudio upstream:

   This is an ALSA kernel driver issue. Please upgrade to newest ALSA and 
   if problem persists file a bug to ALSA upstream.

Kernel people: this problem occurs from 2.6.23 to 2.6.25-rc7, all using 
the Debian packages either from sid or from the kernel team's 
repository.  The driver I'm using is snd-hda-intel, and pulseaudio is 
using the dmix:0 device.  Note that if the driver is not in use by 
pulseaudio, this does not occur.
