At almost every boot PulseAudio starts with the volume muted. The only
time this does not happen seems to be when I don't do a clean log out
(hit the reset button).

This is basically bug 525572 again, but as that problem seems to have
been solved for at least some people I'm filing it as a new bug.

Removing ~/.pulse and rebooting does not make any difference.

I'm attaching a verbose log of PA starting in a muted state.
