This should be gnome-keyring prompting you for the password, not
nm-applet itself.

Stupid question, but have you supplied the correct password to
gnome-keyring?
Do other applications work correctly with gnome-keyring?
Could you try with a fresh keyring resp. fresh account?

Is there anything suspicious in the NetworkManager log (run it as
NetworkManager --no-daemon) or in the users .xsession-errors file?
