I think I've figured it out. nm-applet/gnome-keyring are
prompting for my "default" password but the keys are
actually stored in my "login" keyring. The passphrase for
that appears to be an old login password, probably what my
/etc/shadow password was when I first logged in (it's not
my current /etc/shadow password).

So now I try to figure out why it's asking for one keyring,
actually accessing another, and why gnome-keyring has
stored an old password rather than rely on pam :/

(I'll reassign/split etc. the bug as necessary)
