Asterisk 1.2.9.1 and friends: "Ring requested on channel 0/3 already in use on span 3. Hanging up owner."

Hello,

Just a follow-up on the topic of Asterisk-1.2.9.1 packaging.  We just
had a PBX crash where it refused to accept incoming phone calls from the
PRI and printed this error message:

"Ring requested on channel 0/3 already in use on span 3. Hanging up owner."

I've found reports of this happening to other people in other versions,
so it might just be a coincidence.. but I'm posting here just in case
someone else notices the same thing.
