You need to set up an initial manager user for destar. Though it is
probably a bug that the package does not do that on its own.

After the initial setup, destar will rewrite manager.conf anyway, so
this should not be an issue.

BTW: any reason you don't use a Lenny system?

IIRC the package should use that and set
/etc/asterisk/manager.d/destar.conf
