There should be now the warning I mentioned.
And the support for nodefault was fixed. Note that you will need to add
'<programlisting> <screen>' to the break list.

The options I recommend for your use case are:

-o nodefault='<programlisting> <screen> <replaceable>'
-o untranslated='<programlisting> <screen> <replaceable>'
-o break='<programlisting> <screen>'
-o inline='<replaceable>'


I also think you should use "-o includeexternal -m book.xml" instead of
adding all the files to your command line. This might be easier for later
maintenance.
This unfortunately require that you run po4a from the directory where
book.xml is.
