Setting SANE_DEBUG_MICROTEK=128 reveals the following when executing xscanimage (the last lines of text):

Segmentation fault

I have tried the calibrate options:

norealcal
#noprecal

..in microtek.conf and run xscanimage.  The ready light does out on the scanner.  It seems to be in the recalibration but enabling norealcal makes the scanner not ready while disabling norealcal causes a seg fault.

Clues anyone?
