I don't know if this is the optimal way, byt I've done once before by
downloading d-i sources,
changing the SYSLINUX (ISOLINUX) configuration,
and creating a new d-i image with those modifications. After that,
just add (If I remember well)

custom_installer = PATH_to_new_d-i

in your profile's conf file to indicate the new d-i image.
