Regarding the second part of the bug report, I think it is the same as
http://bugs.debian.org/162842 or http://bugs.debian.org/162303 (this last
one is already closed).

This was fixed in 4.0.3-4.

Note for Tomasz (the upstream shadow maintainer): you may want to use the
patch provided in http://bugs.debian.org/162303
